The Source

The Source is a community clinic based in the Horn Park neighbourhood.
It's staffed by an advanced clinical practitioner nurse who supports the management of long term conditions. Their support includes reviewing blood pressure, blood glucose checks and advice, basic wound care including suture removal and clip removal, basic medication advice and minor ailment reviews.
The Source also provides open access to contraception services and signposting to specialist sexual health services as appropriate (e.g. Market St Clinic, CASH).
Our team
We are a service staffed by:
Tuesday and Thursday mornings: An advanced clinical practitioner nurse, specialising in long term conditions
Tuesday afternoon: Sexual health clinician from CASH / Metro Charity
Tuesday and Thursday afternoons: Live Well coaches from Charlton Athletic Community Trust.
Our aims
We aim to provide access and signposting to community healthcare services closer to home for residents of the Horn Park Estate and surrounding area.
How to access this service
To access this service you must:
- Live in the Horn Park neighbourhood, the Greenwich borough or registered with a Greenwich GP
- Be aged 18 or above (except our sexual health services)
The service is for people who:
- Have long term conditions - Tuesday and Thursday morning clinics are with an advanced nurse practitioner
- Have sexual health concerns - Tuesday afternoon clinics are for contraceptive and sexual health services. We will see anyone in line with the national guidelines. There is no age restriction and residents outside the Greenwich borough will be signposted to their home test provider.
Referrals
No referral is needed, this is a drop-in service and patients are assessed in order of arrival.
How to contact this service
The Source is open Tuesdays and Thursdays.
From 9am to 1pm residents can see an Advanced Nurse Practitioner specialising in long term conditions, with the last walk in at 12.30pm.
Our sexual health service is open Tuesday afternoons.
From 1pm to 4.30pm you can drop into our sexual health clinic. For those requiring sexual health and contraceptive services please contact Greenwich Sexual Health on 020 8301 8920.
