District Nursing Service - Greenwich

  • The District Nursing Service provides planned and unplanned care for adults aged 18 and over who cannot leave their home. Care is offered 24 hours a day, 7 days a week.

    This service covers the following types of care:

    • Wound prevention, assessment and management
    • Post-operative care
    • Medications management
    • Long-term conditions management
    • Stoma care
    • Catheter care
    • End of Life/palliative care, including care for terminally ill patients
    • Bereavement visits and referrals to other agencies
    • Health education, promotion and advice about how to care for yourself

Our aims

We aim to help you live as independently as possible and work together with you, your family, carers and other health and social care professionals. We always treat you as an individual and create a care plan that meets your needs.

How to access this service

To access the service, patients must be registered with a GP in the borough of Greenwich.

 

Referrals

You can self-refer to the service by calling the Single Point of Access Team on 020 8320 3550.

You can also be referred by:

  • Your GP or Practice Nurse
  • The hospital
  • Health and social care professionals
  • Family or carers
