Diabetes Service - Bexley

  • The Bexley Diabetes service works with people with Type Two Diabetes (and people with Type One Diabetes who are housebound).

    Our aims

    We aim to create a plan that improves your ability to manage your diabetes.

    How to access this service

    To access this service, you must be:

    • Registered with a Bexley GP. This includes patients living in Nursing Care Homes and residential care homes
    • Aged 18 or above
    • Diagnosed with Type Two Diabetes by a GP or secondary care
    • Housebound - if you have been diagnosed with Type 1 Diabetes
    • On maximum tolerated oral therapy (typically three oral agents)
    • Suffering from poor glycaemic control despite insulin or GLP-1 mimetic therapy or requiring initiation of insulin or GLP-1 mimetic
    • Hypoglycaemia unaware

Referrals

You can be referred to this service by:

  • Your GP
  • Consultants
  • Secondary care hospitals
  • Other healthcare professionals

We do not accept referrals for:

  • Newly diagnosed Type 2 Diabetes
  • Structured education programmes
  • Titration of oral medications

Patients, carers and advocates can contact the service for advice but cannot refer individuals to the service.

A specialist diabetes nurse assesses every referral based on the information provided in the referral form. If suited to the service, you are offered an initial telephone triage. This is followed by either a clinic appointment or a home visit.
